Impossible and Suicide work, but pressing the 4 LORD keys only gives me a speaker beep (like pressed to many keys at once).
Any more detailed explanations on how to activate the Lord Of The Game difficulty available here? Thx in advance!

K after a few tries I got it. The trick is to follow the instruction, but roll your fingers over L O R D counterclockwise on the keyboard VERY quickly MULTIPLE times until it comes up.
or press them one after the other in groups VERY quickly. LO then RD VERY quickly multiple times until it comes up.

Dosn't work here. Dunno if I'm too slow or something :/
the Cursor is above Suicide. I decreased DosBox' Cycles to 2500 so my typings "arrive slower". I even created a macro on my keyboard that types the letters L, O, R and D in 10 milliseconds spaces.
So you hit the Caps Lock and then fiddle over the l, o, r and d keys or do you press shift and then fiddle over the l, o, r and d keys?
What should happen by the way? Just like Impossible and Suicide the Lord of the Game tag appears?

So you hit the Caps Lock and then fiddle over the l, o, r and d keys or do you press shift and then fiddle over the l, o, r and d keys? You shouldn't need either Caps Lock or Shift... lowercase works just fine here (using DOSBox).

Oki, so I just hit the 4 keys one after the other or press l and hold it, press o and hold it, press r and hold it and press d and hold it? Unfortuantely there's a speaker beep as soon as I hold the third key ...
Is there any sound confirmation or something at this difficulty activation?

You hold L, O, R, and D all at the same time, there's no confirmation (aside from "Lord of Game" showing up.) I used to have similar problems (in game, not in the menu), they seem to have gone away after getting a USB keyboard (which perhaps has no too-many-keys-at-once limit.)

I see. Seems I need to get an USB-Keybord then. Wonder why my macro doesn't work, hm :/

LO RD worked here. I hammered the keys.
Nobody said getting Lord of Game difficult would be easy!
Ha.

My l, o, r and d keys are almost hammered through the keyboard - no go :/

Yeah I just smash the keys rapidly until it registers.

YEAY - I slammed for like 2 minutes the keys: nothin' worked. Then I hit Caps Lock so the lamp lighted up and slammed the 4 keys for another 2 mintes. Finally "Lord of Game" appeared but I thought my fingers were pressing l o d r instead of l o r d. Well, I saved the game with this difficulty level and will now try to reproduce this. An easier key combo would be way better though

well now that we have the source >:-)
It pains me that they make difficulty levels hidden. Kinda stupid.

Well, as the Quake 1 CD instructions says, 'Hidden so no one is foolish enough to wander into it by accident.' (Regarding Nightmare difficulty XD)
